Report of Volunteer Hours

We could not offer Care Team services without the generous gift. You are the backbone of our organization.

A vital part of volunteering is keeping track of the time you spend. This allows us to show our donors and grantors just how much our Care Teams do to change the lives of our Care Partners.

It only takes a minute. All we need is an estimate of all the time that you generously volunteer over the course of a month. This includes (but is not limited to) time spent doing the following things:

• Travel to and from your Care Partner's home
• Communication with your Care Team and Care Team Leader
• The actual time spent doing the tasks for our Care Partners
• Volunteer trainings and information seminars
• Any time you spend getting ready to volunteer
